Medical coverage is administered by BlueCross BlueShield of TN (BCBST) under the “P” network. Two plan design options are available (PPO or HRA).
HRA Funding is administered by BlueCross BlueShield of TN for those that participate in the BCBST HRA plan design. BCBST is set up on an automatic HRA reimbursement to the provider. Participants may opt out by calling BCBST at the number provided on your insurance card.
Dental coverage is available through Delta Dental. Delta Dental is provided under a dual network PPO and Premier.
Vision coverage is administered by VSP.
Flexible Spending Accounts (Medical and Dependent Care) are administered by WageWorks. An annual election is required. The minimum election amount is $250.00. The maximum election amount is $2500.00 for Medical Reimbursement and $5000.00 for Dependent Care. A Wageworks
Long Term Disability is administered by Cigna. Premium is paid by the City.
Basic Life Insurance is administered by Minnesota Life. Premium is paid by the City. Benefits are two times the annual budgeted salary rounded to the next $1000.00. AD & D benefits are provided at an additional two times the annual budgeted salary.
Voluntary Life Insurance is administered by Minnesota Life. This is an additional employee paid benefit for the employee,
Retirement is administered by ICMA-RC. ICMA-RC provides benefits through 401 and 457 plans. There is a mandatory 3% participation in the 401 plan. The City will match up to 8%.
Employee Assistance Program (EAP) is administered by LifeServices. Benefit eligible employees and their dependents can each receive 6 visits per episode each year. Eligible employees do not have to be enrolled
