Notify your supervisor and seek medical care as needed. For an emergency, call 911.  Your supervisor will complete a C-20 form.

31 days from the day of the event.

3. What age does voluntary life coverage stop for children?

Eligibility for voluntary coverage ends at age 26, and employees are responsible for notifying the Employee Services Department to stop coverage.
